Parent/Guardian/Custodian should provide the following documents when enrolling the student:

1. Proof of Immunization
2. Birth Certificate
3. Two Proofs of Legal Residence (see below for specifics on two proofs)

  • Lease/Rental Agreement
• 1 Proof of Utility at Legal Residence
  • Current Power Bill
  • Current Gas Bill
  • Current Telephone Bill
